About
Operator first.
Sarge the NEMT Doc is the operator brand of Garfield Warren — NEMT company founder since 2017, and a certified trainer for both the Community Transportation Association of America (CTAA) and the Non-Emergency Medical Transportation Accreditation Commission (NEMTAC).
He built his NEMT company the way most owners do: hands-on, in the trenches, learning which broker contracts actually pay, which documentation habits survive audits, and which "expert advice" falls apart the first time a real trip gets real. The coaching practice exists because the gap between launching and operating is where most NEMT businesses stall — and scattered checklists do not close it.

The method: Diagnose, Prescribe, Scale
Every engagement runs the same sequence Sarge uses on his own operation. Diagnose — model, gaps, and priorities, separating urgent decisions from distractions. Prescribe — setup, systems, and a weekly action plan with documented decisions. Scale — demand, unit economics, service capacity, and the metrics that decide what comes next. It is the spine of the 6-Week NEMT Launch Accelerator and of the weekly industry intelligence he publishes.
Where the intelligence comes from
The articles on this site are built on the same daily scan behind the Axis Intel briefing: state Medicaid bulletins, broker transition announcements, audit reports, training calendars, and software moves — cross-checked against what is actually happening on the street. When the broker map reshuffles, you read it here with the operator's angle attached: what it means for your volume, your credentialing, and your next move.
