The 2026 NEMT Broker Map: Who Controls Medicaid Rides in Your State

Updated November 4, 2026 · by Sarge the NEMT Doc · 8 min read

If you run non-emergency medical transportation, one fact sits above everything else: you do not choose your customers — your broker does. Brokers are the companies states and Medicaid managed-care plans hire to assign trips, set rates, credential providers, and audit documentation. When your broker changes and you are not re-credentialed in time, your revenue reroutes to someone else without ceremony.

And 2026 has been the most disruptive broker year in recent memory. If you have not checked your state's map lately, assume it moved.

The brokers that run NEMT

Five names control the vast majority of Medicaid NEMT trips in the United States:

  • MTM Health — the largest broker by statewide contracts and the most aggressive expander in 2026.
  • Modivcare — the legacy giant, still holding more statewide contracts than anyone but defending several under rebid and audit.
  • SafeRide Health — strong with managed-care plans, including Molina markets.
  • MediDrive — Colorado's new statewide broker and the biggest single-state consolidation of the year.
  • Alivi — active in several states with plan-specific contracts.

Confirmed 2026 broker moves

These are transitions with announced go-live dates — not rumors. This table is your early-warning system:

EffectiveState / PlanMove
Oct 1, 2026Texas — Blue Cross Blue ShieldFlipped to MTM Health
Oct 1, 2026Virginia — Medicaid fee-for-serviceMoved to MTM Health
Oct 1, 2026Montana — statewideAwarded to Modivcare
Oct 1, 2026Illinois — Blue Cross Blue ShieldFlipped to MTM Health
Q4 2026Michigan — MolinaMoved to SafeRide Health
Nov 1, 2026New Mexico — Blue Cross MedicaidNow booking through MTM Health
Dec 1, 2026Colorado — Health First ColoradoMediDrive opens member booking (app, web, phone) for Jan 1+ appointments
Dec 31, 2026Washington, D.C.MTM switch held through year-end amid provider protest; monitor for the new go-live
Jan 1, 2027Colorado — statewide mandateEvery Health First Colorado trip must flow through MediDrive — zero direct state billing
Nov 6, 2026Michigan — $160.5M statewide rebidRFP closes; incumbent Modivcare is being audited simultaneously by the state auditor

The pattern to notice: every one of these moves strands providers who wait. When Virginia's fee-for-service moved to MTM on October 1, providers running only Modivcare credentials stopped receiving assigned volume that morning. The winners had re-credentialing completed weeks before go-live.

How to find your state's current broker

The broker map is a moving target, and no public list stays current for long — which is exactly why you verify it yourself, quarterly:

  1. Your state Medicaid agency's provider page. Search "[your state] Medicaid NEMT broker" — the agency publishes the current transportation broker arrangement.
  2. Each MCO's provider manual. In managed-care states, the plan — not the state — picks the broker. If you serve Molina, Humana, or BCBS members, pull that plan's NEMT section.
  3. The broker portals themselves. MTM, Modivcare, SafeRide, and MediDrive all maintain provider portals with state-by-state participation details.

How to get contracted with a NEMT broker

Credentialing is infrastructure, not paperwork. Here is the sequence I walk operators through:

  1. Confirm your state Medicaid enrollment first. Brokers contract enrolled providers. No state enrollment, no contract.
  2. Register with the broker's credentialing platform. MTM and several others route through ProCredEx; each broker also maintains its own provider portal.
  3. Assemble the document packet: commercial insurance (auto and GL/garage), vehicle list with inspections, driver qualification files, safety and training policies, and your dispatch/records workflow.
  4. Pass the readiness review. Expect document verification and, in some states, an operational review of your trip-documentation process.
  5. Keep every credential current. Continuous monitoring is now the audit standard — an expired credential can mean recoupment of every trip it touched, not just a slap on the wrist.

Your broker switched: the survival checklist

  • Confirm the announced go-live date and your current credential status with the new broker — today, not next week.
  • Start portal registration and document upload the same week the switch is announced.
  • Do not cancel your old broker contract until the new one is active and trip flow is confirmed.
  • Brief your dispatch team on the new portal's trip-adjudication workflow before the first trip.
  • Calendar a 30-day post-switch review: volume, denial reasons, documentation gaps.

Frequently asked questions

What is a NEMT broker?

A company hired by a state Medicaid program or managed-care plan to manage non-emergency medical transportation. The broker assigns trips to contracted providers, sets rates, runs credentialing, and audits trip documentation. No broker contract in your service area generally means no Medicaid trips.

Who is the largest NEMT broker in 2026?

MTM Health and Modivcare lead by statewide contract count, with SafeRide Health, Alivi, and Colorado's MediDrive covering major regions. Your broker depends on your state — and sometimes your specific health plan.

What happens to my trips when my broker switches?

Your assigned volume moves to the new broker — but only after you complete that broker's credentialing. Miss the window and your trips go to contracted providers instead of you, with no automatic carryover.

How long does broker credentialing take?

Plan for several weeks from portal registration to active status. The moment a transition is announced is the moment the clock starts — not the go-live date.

Do I need a broker contract to get paid for Medicaid trips?

In most states, yes — Medicaid NEMT runs through closed broker or plan networks. Check your state Medicaid agency's provider page for the current arrangement.

Is your operation broker-proof?
